Photograph: Facebook<\/p>\n\n\n\n<p>Advocates fear such ads could roll back decades of hard-won progress against HIV\/Aids and are calling on <a href=\"http:\/\/www.facebook.com\">Facebook<\/a> to change its policies<\/p>\n\n\n\n<p>Since late summer, many LGBTQ+&nbsp;<a href=\"https:\/\/www.theguardian.com\/technology\/facebook\">Facebook<\/a>&nbsp;users\u2019 newsfeeds have begun to display medically incorrect targeted advertising. These ads pertain to Truvada, a one-pill-a-day pharmaceutical that has been demonstrated to reduce the likelihood of HIV transmissions by as much as 99%, making it a key mechanism in the decades-long fight against HIV\/Aids.<\/p>\n\n\n\n<p>\u201cSide Effects from taking an HIV Drug \u2026\u201d reads one badly punctuated message, full of random capitalizations. \u201cThe manufacturers had a safer drug &amp; kept it secret \u2026 They kept selling the dangerous one.\u201d<\/p>\n\n\n\n<p>That ad, which runs on Instagram as well as Facebook, is paid for by the Virginia law firm&nbsp;<a href=\"https:\/\/get.kbaattorneys.com\/\">KBA Attorneys<\/a>. It cites unspecified bone and kidney conditions as side-effects from Truvada, dangling the prospect of financial compensation from what appears to be a nascent product-liability lawsuit against manufacturer Gilead Sciences.<\/p>\n\n\n\n<p>Advocates took note.<\/p>\n\n\n\n<p>\u201cPrEP is safe and generally well-tolerated,\u201d says Trevor Hoppe, a sociologist of sexualty, medicine and the law. \u201cAny misinformation to the contrary is likely bad for public health, especially communities hardest hit like gay men in the US.\u201d<\/p>\n\n\n\n<p>Addressing the ad\u2019s claim of bone damage, the San Francisco Aids Foundation&nbsp;<a href=\"https:\/\/www.sfaf.org\/collections\/beta\/truvada-lawsuit-ads-are-misleading\/\">says<\/a>&nbsp;Truvada\u2019s effects are \u201cnot clinically significant\u201d, adding that it \u201chas been shown to cause a 1% decrease in bone mineral density, a change that reverses once the medication is stopped.\u201d<\/p>\n\n\n\n<p>The most forceful response came in the form of an&nbsp;<a href=\"https:\/\/www.glaad.org\/blog\/open-letter-facebook\">open letter<\/a>&nbsp;that the LGBTQ+ advocacy organization Glaad published on Monday, 9 December, co-signed by more than 50 prominent LGBTQ+ and public-health organizations, with politicians like Governor Andrew Cuomo of New York and Senator Elizabeth Warren of Massachusetts subsequently joining. Calling on the social-media giant to remove the misleading ads, the signatories demanded that Facebook commit to \u201ca review and potential update of current advertising policies to prevent false or misleading public health statements from reaching users\u201d.<\/p>\n\n\n\n<p>That reach may be quite extensive.<\/p>\n\n\n\n<p>\u201cThere\u2019s definitely more than one ad,\u201d says Rich Ferraro, the chief communications officer for the LGBTQ+ advocacy organization Glaad. \u201cThere\u2019s a range of personal-injury law firms running them, and there were millions of views on some of them.\u201d<\/p>\n\n\n\n<p>Using the&nbsp;<a href=\"https:\/\/www.facebook.com\/ads\/library\/?active_status=all&amp;ad_type=all&amp;country=US&amp;impression_search_field=has_impressions_lifetime\">Facebook Ad Library<\/a>, the Washington Post determined that six anti-PrEP ads from personal injury firms Lawsuit Watch and Advocate Alliance Group got as many as 1.3m views on Facebook.<\/p>\n\n\n\n<p>\u201cDoctors were citing these ads as reasons why people who should be on PrEP were not,\u201d Ferraro says. \u201cWhat makes them so unique is the targeting ability. There\u2019s a drought of information about HIV-prevention in mainstream media and even in LGBT media, and so at-risk people are seeing them in their newsfeed.\u201d<\/p>\n\n\n\n<p>The risk here isn\u2019t so much that people may be misinformed as that it could roll back decades of hard-won progress against HIV\/Aids. In working to transform the disease from a death sentence to a chronic condition that can be managed with appropriate treatment, public health advocates have worked tirelessly to establish trust and bring hard-to-reach populations like sex workers and intravenous drug users into the system.<\/p>\n\n\n\n<p>\u201c<a href=\"https:\/\/www.gettingtozerosf.org\/\">Getting to Zero<\/a>\u201d has long been the mantra, a number that refers to zero new transmissions, zero deaths from complications arising from HIV\/Aids and zero stigma. In San Francisco, the number of new infections citywide went from a recent peak of 453 in 2012 to a low of 197 last year. Organizations such as the San Francisco Aids Foundation have cited PrEP as a significant contributor to this success, always with the caveat that progress is fragile. Racial disparities remain, and hard-to-reach populations are often the first to fall out of care.<\/p>\n\n\n\n<figure class=\"wp-block-image\"><img decoding=\"async\" src=\"https:\/\/i.guim.co.uk\/img\/media\/1025495056853dfa3f71d33f79c5cd412fcad605\/0_102_3000_1800\/master\/3000.jpg?width=300&amp;quality=85&amp;auto=format&amp;fit=max&amp;s=81edef82947b593990f0ecbf8a684ef0\" alt=\"Truvada is a key mechanism in the decades-long fight against HIV\/Aids.\n\nFacebook ads are spreading lies about anti-HIV drug PrEP. Photograph: Justin Sullivan\/Getty Images<\/figcaption><\/figure>\n\n\n\n<p>That Facebook tolerates debunked claims on its platforms shouldn\u2019t really be a surprise. After all, the company has publicly disavowed any responsibility to factcheck political advertising, a profit-first decision reached despite&nbsp;<a href=\"https:\/\/www.theguardian.com\/technology\/2019\/oct\/28\/facebook-employees-strongly-object-to-policy-allowing-false-claims-in-political-ads\">widespread internal dissent<\/a>.<\/p>\n\n\n\n<p>But they\u2019re not always consistent, either. Peter Staley of PrEP4All Collaboration noted that the company doesn\u2019t always a take hands-off approach regarding the veracity of ads. For instance, the company&nbsp;<a href=\"https:\/\/www.out.com\/health\/2019\/10\/30\/facebook-wont-ban-fake-news-they-will-ban-prep-ads\">prohibited<\/a>&nbsp;a New York medical provider that works with Asian and Pacific Islanders from raising awareness about PrEP in those communities.<\/p>\n\n\n\n<p>At the same time, Ferraro notes, Glaad\u2019s relationship with the company has been quite strong. In 2010, the organization helped established Facebook\u2019s&nbsp;<a href=\"https:\/\/www.facebook.com\/notes\/facebook-safety\/facebooks-network-of-support\/161164070571050\/\">Network of Support<\/a>, which was instrumental in combatting online bullying and expanding the site\u2019s gender-identity options. In an echo of the present controversy, they also worked together in 2018 to remove ads for so-called conversion therapy, something Ferraro calls \u201ca dangerous and debunked practice\u201d.<\/p>\n\n\n\n<p>This time might be different because of the way Facebook now evaluates truths and falsehoods. When Glaad contacted the company\u2019s public-policy team, they were redirected to a public-facing advertising policy page that lists approximately a dozen reasons why Facebook would remove ads, including \u201cmisinformation\u201d.<\/p>\n\n\n\n<p>\u201cThey\u2019ll take them down when experts question things,\u201d Ferraro says, adding that another avenue is through third-party factchecking agencies.<\/p>\n\n\n\n<p>\u201cThere are six of them in the United States,\u201d he adds. \u201cWe contacted five.\u201d<\/p>\n\n\n\n<p>The sixth is a&nbsp;<a href=\"https:\/\/www.theguardian.com\/technology\/2019\/apr\/17\/facebook-teams-with-rightwing-daily-caller-in-factchecking-program\">partnership<\/a>&nbsp;with a subsidiary of the website the Daily Caller, an \u201calt-lite\u201d site founded by Tucker Carlson and known for&nbsp;<a href=\"https:\/\/www.rawstory.com\/2015\/02\/daily-caller-embarrasses-itself-with-kill-all-the-jews-headline-but-no-one-should-be-surprised\/\">deliberately outrageous headlines<\/a>, spuriously&nbsp;<a href=\"https:\/\/dailycaller.com\/2013\/11\/05\/gays-now-totally-boring-enda-struggle-reveals\/\">trolling LGBTQ+ people<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/dailycaller.com\/2019\/05\/24\/hhs-obama-era-transgender-hhs\/\">referring to trans rights<\/a>&nbsp;as \u201cspecial treatment\u201d.<\/p>\n\n\n\n<p>Needless to say, Glaad would not be satisfied with that website weighing in.<\/p>\n\n\n\n<p>\u201cThe fact that the Daily Caller is potentially making a call on ads that deal with issues of HIV prevention is something Facebook should not hide behind,\u201d Ferraro says.<\/p>\n\n\n\n<p>Facebook did not return a request for comment. In the absence of a change in corporate policy, Glaad has decided to purchase an ad buy to promote its open letter, in the hopes that LGBTQ+ users read and share it.<\/p>\n\n\n\n<p>\u201cIf they\u2019re going to continue profiting off harming public health, our community is going to call them out,\u201d Ferraro says. \u201cAs much as they support their LGBT employees, their platforms have become places where anti-LGBTQ organizations can do serious damage. Instead of trying to educate their audience, they are instead helping the spread of HIV by sitting idly by.\u201d<\/p>\n\n\n\n<p>It is the opacity and the lack of accountability that infuriates advocates as much as the false ads themselves \u2013 which, incidentally, continue to run.<\/p>\n\n\n\n<p>This is just the latest front in the low-intensity conflict with the LGBTQ+ community that Facebook has waged for the better part of this decade. Drag queens\u00a0<a href=\"https:\/\/www.salon.com\/2014\/09\/24\/let_me_be_lil_miss_hot_mess_facebook_took_away_my_stage_name\/\">led the fight<\/a>\u00a0against the company\u2019s \u201creal-name\u201d policy, which was seen as transphobic and potentially dangerous to individuals who may not be out (as well as for domestic-violence survivors). Its use of identifying ad data may\u00a0<a href=\"https:\/\/www.newscientist.com\/article\/2214309-facebooks-ad-data-may-put-millions-of-gay-people-at-risk\/\">jeopardize the safety<\/a>\u00a0of queer users in countries like Saudi Arabia, where homosexuality is punishable by death. There is a widespread perception that the prudish <a href=\"https:\/\/www.dazzlepop.net\/site\/category\/social-media\/\">platform<\/a> is\u00a0<a href=\"https:\/\/www.change.org\/p\/lgbtq-stop-instagram-from-doing-homophobic-and-sexist-actions\">especially censorious<\/a>\u00a0regarding same-sex content while\u00a0<a href=\"https:\/\/projects.propublica.org\/graphics\/facebook-hate\">remaining oddly lax<\/a>\u00a0when policing homophobic hate speech.<\/p>\n\n\n\n<p>To be fair, however, criticism of PrEP hasn\u2019t come exclusively from manipulative attorneys or even the homophobic quarters of the political right. Michael Weinstein of the Aids Healthcare Foundation famously&nbsp;<a href=\"https:\/\/www.nytimes.com\/2017\/04\/26\/magazine\/the-ceo-of-hiv.html\">dismissed<\/a>&nbsp;it as a \u201cparty drug\u201d, implying that it\u2019s some kind of intoxicant. Weinstein\u2019s underlying point \u2013 that Truvada will reduce condom usage, leading to an increase in other sexually transmitted infections \u2013 has a kernel of truth to it, but there are ways to address unintended consequences without delegitimizing medical science.<\/p>\n\n\n\n<p>In a climate where trust in expertise is already dangerously low, the spread of doubt represents a big step backward in the fight against the greatest public health crisis of the past half-century.
